Over my lifetime as a surgical nurse, I have seen medical advancements such as using staples instead of sutures for skin closures, devices that cut and stable all in one squeeze of a handle, and we now practice medicine where the surgeon can work from across the room using robotic arms through small incisions. As medical techniques continue to advance, one thing has not progressed, the use of the rolled towel to support our instruments. My mother is 89 years old and remembers working off a rolled towel. While it seems to be such a minor thing, how we organize and deliver instruments back and forth from the field to the table, has a direct impact on the safety of the patient and the staff.
” In Queue” allows instruments to stay upright so that they easily can be passed back and forth, no matter what the pace of the surgery is. This allows time for instruments to be cleaned and placed back on “In Queue” after each use without causing hardship to the team. Cleaning at point of use is one of the first lines of defense in preventing infections. “In Queue” provides consistency in the organization of instruments on the field; this is especially important when there is a changeover in personnel during a case. Because instruments are upright and organized personnel can easily do an assessment of where their instruments are at any given time during the case. When you are driving you are constantly aware of your surroundings, not just what is in front of you. Navigation of surgical instruments should be along the same lines, you should always know where things are on the field, not just at the time of counting. This not only helps prevent “never events” such as retained instruments but also helps to prevent punctures wounds to staff and patients.
“In Queue” allows a quick set up, simply place “In Queue” on your table and or mayo and remove the stringer rod. At the end of the case replace the stringer rod in seconds and you are ready to send instruments back down to sterile processing for cleaning.
In sterile processing “In Queue” provides an efficient method to arrange instruments to assure accurate counts. “In Queue” allows instruments to be processed in the automatic washer/disinfector in an organized manner with tips up and instruments open to their fullest to ensure accurate cleaning. During sterilization “In Queue” keeps tips open to ensure instruments are sterile.
“In Queue” bridges the gap between Surgery and Sterile Processing by providing an organized method for instruments to move to and from the two areas, increasing efficiency in which the departments can work. Having an organized method to handle instruments helps prevent instrument damage and loss.
